Indometacin (Indometacinum, Indobene, Metindol). One of the most effective NSAIDs. It has a prominent analgesic activity. It’s a strong inhibitor of prostaglandin biosynthesis.
Pharmacokynetics. Bioavailability – 85-98%, t1|2 4-9h., it slowly penetrates into the cavities of joints; it undergoes biotransformation in liver, get excreted mainly by kidneys.
Indications: rheumatoid arthritis, periarthritis, ankylosing spondylitis, osteoarthritis, gout, thrombophlebitis and other diseases accompanied by inflammation. It is also used for nephrotic syndrome.
Used orally, i/m, rectally and locally.
- Orally (after meal) it is prescribed usually starting with 0,025 g (25mg) 2-3 times a day, then, depending on how a patient tolerates the drug, the daily dose is increased up to 0,1-0,15g (100-150 mg) in 3-4 intakes.
- Extended-release Indometacin (extended-release Metindol) are taken starting with 0,075 g (1 capsule) 1-2 times a day (in the morning and evening) in the course of 5-10 days; then 1 capsule a day (in the evening).
- Rectal suppositories of 0,05 or 0,1g are prescribed 2 times a day.
- For the treatment of acute states it is injected i/m – 0,06 g 1-2 times a day in the course of 7-14 days. After that the injections should be substituted by tablets or suppositories.
Indometacin ointment is used for the infriction incase of acute and chronic polyarthritis, neuritis, plexitis, radiculitis, thrombophlebitis, psoriasis arthropica.
Total amount of ointment per day should not exceed 15 sm squeezed out of the tube.
Side effects: headache, dizziness, sleepinsee. Sometimes one observes nausea, vomiting, anorexia, pains in epigastric region, constipations. The drug can cause an ulcerogenic action in GIT, bronchospasm, arrhythmia, toxic hepatitis, edema, increase of AP, depression of blood formation.
Contraindications: gastric and duodenal ulcers, ulcerative processes in intestines and esophagus, thrombocytopenia, BA, renal failure. The drug mustn’t be prescribed for pregnant women.
The manifestations of dizziness are possible. It is especially crucial for drivers, people working with any kind of machinery.
