Ibuprofen

Ibuprofen (Ibuprofenum, Brufen, Nurofen). 

It has anti-inflammatory, analgesic and moderate antipyrietic activity. By its potency it is a little less effective than Ortophen and Indometacin, but it’s tolerated by patients much better.   

Indications: rheumatoid arthritis, deforming osteoarthrosis (spondylosis), ankylosing spondylitis and various forms of articular and extra-articular rheumatoid diseases as well as in case of pain syndrome at some inflammatory disturbances of peripheral nervous system.

Pharmacokinetics. Taken orally, it is quickly absorbed, t1|2 2-2,5 h (extended-release tab. – 12 h). It penetrates the cavities of joints slowly, but it stays in the synovial tissue in high concentrations; it undergoes biotransformation and is excreted by kidneys in the form of metabolites.  
It is
taken orally and externally. 
  • Orally it is prescribed for adults – 0,2-0,8 g 3-4 times a day. When the therapeutic effect is reached, the daily dose must be reduced down to 0,6-0,8 g.
  • The first day-time dose should be taken in the morning before meal (for the fast absorption) drinking tea after, while all the rest doses – in the course of the day after meal (for the gradual absorption).  
  • Gel and cream is applied onto the involved region 3-4 times a day. 
Contraindications: acute ulcers and exacerbations of gastric and duodenal ulcer disease, ulcerative colitis, as well as optic nerve disease, depression of blood formation, hemorrhagic diathesis, portal and arterial hypertension, heart failure and last trimester of pregnancy. 

With caution: for the patients have already had gastric or duodenal ulcers before; gastritis; enteritis; colitis; chronic hepatitis; liver cirrhosis; arterial hypertension; chronic heart failure, age under 12. 

Interaction. It decreases antihypertensive effect of  ACE (angiotensin converting enzyme) inhibitors and natriuretic activity of Furosemide and Hydrochlorothiazide, increases the risk of hemorrhagic complications if taken in combination with anticoagulants of coumarin row, glucocorticosteroids and alcohol.   

Dosage forms: tab. – 0,2; 0,4 and 0,6 g; dragee – 0,2 g; extended-release tab. – 0,8 g; extended-release capsules – 0,3 g ; 2% syrup in 100 and 200ml bottles; 4% solution (drops) in 15 ml bottles; 5% cream in 20, 50 and 100 g tubes; gel in 30 g tubes.
